Please ensure that all players have completed a parental consent form for season 2010/2011. A new form should also be completed if there are any changes to players circumstances.
The consent form confirms parental consent for participation and also the use of photography by the SYFA. It is also required should emergenct medical treatment be required and to alert the SYFA to any allergies or current medical conditions. The form can be downloaded from the home page of the Regional Development Section.
